COVID-19 Medications

• remdesivir
  • loading dose: 200 mg once
  • then
  • maintenance dose (starting 24 hours after loading dose) of 100 mg every 24 hours x 9 days
  • Exclusion criteria:
    • Multiorgan failure
    • Pressor requirement
    • ALT > 5 X upper limit of normal
    • Creatinine clearance <30 ml/min
    • Dialysis
    • Cannot use with another antiviral agent
    • Not sure about any other renal/hepatic adjustment
• tocilizumab
  • Protocol 1
    • 400 mg IV x1
    • (if only avail as prefilled syringes of 162 mg, may give 2.5 syringes, which is 405 mg).
    • Repeat in 24 hrs???
    • Can consider repeating once in 7 days
  • Protocol 2
    • 8mg/kg IV x1
    • then
    • 4mg/kg the next day
    • Unclear if give again at 7 days after first or second dose and, if so, what dose
  • sarilumab
  • siltuximab
